The terms of using Seriva.
This site provides general information about Seriva Health. It is not medical advice and does not create a clinician-patient relationship.

These Terms govern your use of the Seriva website and the services offered by Seriva Health, LLC (“Seriva,” “we”). By creating an account or making a purchase, you agree to these Terms and to our Privacy Policy.
What Seriva is, and is not
Seriva is a technology company. We are not your healthcare provider and not a pharmacy. Clinical services, including evaluation and, where appropriate, prescribing, are provided by an independent professional entity whose licensed clinicians exercise their own medical judgment. We do not practice medicine, and we do not influence clinical decisions.
Not insurance
The service is cash-pay only. We do not bill Medicare, Medicaid, or any insurer. HSA and FSA funds may be usable; confirm with your administrator.
Eligibility
You must be 18 or older and physically located in a state where the service operates, and you must agree to telehealth care and to these Terms. You agree to provide accurate information, including your location, which we may verify.
The clinical relationship and consent
If you proceed to care, you will enter a clinical relationship with the independent professional entity and complete a separate informed consent to telehealth and treatment. That consent covers the asynchronous (store-and-forward) nature of the evaluation and the risks of naltrexone, including the opioid contraindication, the hepatotoxicity warning, and the risk of lost-tolerance overdose after discontinuation, along with emergency instructions. Your health records are governed by that entity's Notice of Privacy Practices.
Medication and pharmacy
Where clinically appropriate, a prescription is sent to your chosen pharmacy. You pay the pharmacy directly for the medication; we do not sell or dispense it.
Fees, billing, cancellation, and refunds
What you pay.A one-time consultation fee covers a licensed clinician's review of your intake. If care is appropriate, you join an ongoing membership for clinician access, refill review, and support. You see the exact price before you start. The membership does not include the cost of the medication, which you pay your pharmacy directly.
Billing. Exact pricing and billing cadence are shown before you start, and any recurring charge renews only until you cancel. We charge the payment method on file at the start of each billing period. Where a recurring charge applies, a separate, unchecked recurring-charge consent showing the price and cadence is presented before payment, and it is logged with a timestamp.
Cancellation. You can cancel anytime, self-serve in your account, with the same ease as sign-up and no retention maze. Cancellation takes effect at the end of the paid period, and you keep access through that period.
Refunds. The consultation fee is non-refundable once the clinician has reviewed your intake. The membership is refundable on a published pro-rata basis if your care is clinically discontinued mid-cycle or you cancel for cause. We always refund double charges, pricing errors, and accidental purchases. Refunds are processed within 30 days, and you have 60 days to dispute a charge.
Price changes. We give advance notice before any renewal at a changed price, with a chance to cancel first.
No medical guarantees; emergencies
We make no guarantee of sobriety, cure, or any specific outcome. The service is not monitored for emergencies. If you have a medical emergency call 911; for mental-health crises call or text 988.
Acceptable use
Do not misuse the service, provide false information, attempt to obtain medication contrary to clinical advice, or infringe our or others' rights.
Intellectual property
Website content and the Seriva marks are owned by Seriva Health, LLC. We grant you a limited, revocable license to use the service for its intended purpose.
Disclaimers and limitation of liability
The technology service is provided “as is.” To the maximum extent permitted by law, Seriva Health, LLC disclaims implied warranties and limits its aggregate liability. Clinical liability rests with the independent professional entity and its clinicians, not with Seriva.
Dispute resolution and governing law
Dispute-resolution, arbitration, and governing-law terms will be stated here before launch.
Changes and contact
We may update these Terms with notice and an updated effective date; material changes affecting recurring charges will be noticed in advance with a chance to cancel. Questions: support@serivahealth.com, Seriva Health, LLC.
